This course will address the answers to specific treatment problems. Tips and comparisons of various Class II techniques on treating Class II cases in the mixed and permanent dentition will be discussed. Participants will look and compare treatment criteria, appliance placement and adjustments and treatment responses for various fixed and functional Class II techniques including, bionators, twin blocks, Forsus, elastics and distalizers. ‘Thin slicing’ extraction cases will also be discussed including, when to take teeth out and which teeth to take out. Treatment criteria for lower incisor extractions and management of congenitally missing teeth in extraction cases will be demonstrated. Dr Lowenhaupt will also share more of the tips and tricks that he has acquired over the last 30+ years of orthodontics. He will discuss relapse cases and what to do about them as well as things that didn’t work. Multiple cases will be utilized to demonstrate various aspects of mechanics, finishing and detailing and various treatment modalities. Dr Eric Lowenhaupt attended Florida State University for undergraduate studies and Emory University in Atlanta for his Dental and Orthodontic training. He completed a general practice residency and practiced general dentistry for six years before returning for his graduate orthodontic training at Emory University. Since 1986, he has been in the private practice of Orthodontics and Dentofacial Orthopedics in Florida. Eric lectures nationally and internationally through his affiliation with (G) nathos (The Ricketts School for Bioprogressive Education, Southeast)and VOCA (Vital Orthodontic Considerations Australasia).

Professor Vasilios Chronopoulos received his Dental degree from the University of Athens, Greece and his certificate in Prosthodontics from Marquette University in Milwaukee, USA. He has a MS and a PhD in Dental Biomaterials from the School of Dentistry, National and Kapodistrian University of Athens, Greece. Over the last two decades Professor Chronopoulos has held several Full time and Honorary Academic positions, including: Professor of Restorative Dentistry, School of Dentistry and Oral Health, Griffith University, Australia, Clinical Professor, Department of Restorative Sciences and Graduate Prosthodontics, Baylor College of Dentistry, Dallas, Texas, USA, Assistant Professor, Department of Prosthodontics, School of Dentistry, National and Kapodistrian University of Athens, Greece, Honorary Professor, Faculty of Dentistry, University of Hong Kong, Hong Kong, China, Adjunct instructor, Department of Periodontics, Endodontics, and Dental Hygiene of University of Louisville, Louisville, Kentucky, USA. He has authored and co-authored over 60 publications, chapters in Prosthodontic text books and a text book on Dental Materials, a textbook on All-Ceramic restorations and a Clinical manual on Restorative Dentistry. He lectures nationally and internationally on Esthetic Dentistry, Implants, Prosthodontics and Dental Materials. He is a member of numerous scientific societies. As the demand for all-ceramic restorations continues to grow, clinicians are being forced away from conventional luting cements and must now be familiar with these new adhesive luting systems. Suddenly, it is no longer possible to just stock one cement for all indirect applications... or is it? Common clinical questions such as “What is a porcelain primer?”; “What is a silane coupling agent?”; “Is it necessary to etch and bond the dentine before using composite cements?”; “How should porcelain inlays be cemented?”; and “What is the best way to cement zirconia and eMax restorations?” will all be addressed. In an age of endless marketing hype, it can be difficult to know which cement is needed for which restoration. Dr Michael Mandikos is a registered specialist in Prosthodontics. He received his Bachelor of Dental Science Degree with Honours, from the University of Queensland and in 1998 he completed a three-year residency program at the State University of New York at Buffalo (USA), graduating with a Certificate in Prosthodontics and Masters Degree in Biomaterials. He has researched composite resin materials and published research papers in Australian and international journals on clinical and dental materials topics, as well as many clinical technique articles in local dental magazines. Michael is a Visiting Specialist Prosthodontist to the University of Queensland Dental School and formerly to the Royal Australian Air Force. He is a reviewer for several Journals including the Australian Dental Journal and Quintessence International and is a product evaluator for several dental companies. Michael has presented continuing education programs at Dental meetings throughout Australia, Southeast Asia and the USA and maintains a Private Practice limited to Implant and Restorative Dentistry in Graceville in Brisbane’s Inner West.

The Hall Technique involves sealing early caries in primary molars with preformed metal crowns with no use of hand pieces or local anaesthesia and no removal of caries. A preformed metal crown is cemented directly over caries that does not extend radiographically beyond the middle third of dentine. The sealing of the tooth in this way prevents the carious lesion from progressing by isolating the bacteria and starving it of oxygen, simple carbohydrates, and an acidic environment. All of which are required for the proliferation of cariogenic bacteria and progression of the carious lesion within the tooth. The Hall Technique is set to revolutionise children's dentistry. This course has been developed and will provide you with all the information you need to incorporate this clinical technique and build your practice, through the non-invasive treatment of childhood caries in primary molars. You will find that this simple technique and anxiety reducing technique can increase your income stream whilst retaining families in your practice with less referrals for treatment under general anaesthesia. Learning outcomes: To understand the evidence for and indications for the Hall Technique, including important diagnostic criteria; To learn the steps involved in the placement of a Hall Technique crown To select appropriate cases for use of the Hall Technique To implement the Hall Technique as part of an MID approach to management of dental caries The course consists of: An on-line training component (approximately 7 hours). The on-line training includes: An overview of the evidence for and current research on the Hall Technique – video presentation with short test. Current thinking on caries management in the primary dentition. Four journal articles about the Hall Technique with verification questions. The diagnosis of caries using the ICDASS (International Caries Detection Assessment System). Radiography – Review of radiographic technique and Diagnosis for appropriate use of the Hall Technique. Guidelines – guideline for the evidence based use of stainless steel crowns in managing carious primary molars. Half-day Pre-clinical hands-on component: This consists of two hours of pre-clinical lab based activities. Benchtop exercise in sizing SSCs and placing separators and Case Discussions, based on slides. Clinical component with supervision: This component comprises of five hours of supervised clinical practice undertaken at the Melbourne Dental Clinic and is designed to build the confidence of the clinician in a supported environment. Participants will undertake assessment and separator placement then the Hall Technique crown placement on their patients at two separate clinical sessions. The first clinical session follows immediately after the pre-clinical (same day), and the second clinical session will take place within 5 days of the first. You will be expected to organise your own suitable patients for these sessions. Credentialing paperwork and patient history forms will need to be submitted ahead of time. Clinical practice in your own clinic for a period of 6 months. A case log book to be kept during this period of time in which you place Hall Technique crowns. At the end of the six months the log book will need to be submitted and reviewed. A Final Case presentation will need to be developed by each participant and posted on the Hall Technique Blog, de-identified, and prepared as a case study, complete with radiographs and clinical photos. In the 6 months following the on-line and practicum course you may use the opportunity of connecting with the Hall Technique community and clinical supervisors on the Blog space that has been set up. You may post questions, comments, observations, photos or short videos – anything that will assist you and others in using the Hall Technique for children. Professor Hanny Calache is a specialist paediatric dentist, recently retired from his role as Director Clinical Leadership, Education and Research at Dental Health Services Victoria. He has undertaken many research projects to improve the state of the public oral health sector, including the first Australian study into the Hall Technique. He has seen much success with his research and in 2011 was recognised with the Victorian Minister for Health’s Public Health Award for Developing a Capable and Engaged Workforce. Dr Martin Hall graduated as a dentist from Adelaide University in 1980. He has worked 37 years in public oral health, remote and rural aboriginal health, international development, research and dental therapist training. He is the Chief Oral Health Advisor for DHSV. Prior to this he was for 20 years the Manager of the Oral Health program and Clinical Director at North Richmond Community Health, where a majority of patients come from a “special needs” background. He is also an investigator on the Hall Technique research study. His innovative health promoting model of care for oral health won the 2013 Public Oral Health Award. Dr Rachel Martin graduated as a dentist from Melbourne University in 1991, and gained a Masters of Public Health in 1998. She has worked 26 years in public oral health, developing community programs, teaching and research. She works as a clinician in the Oral Health program and leads integration of services at North Richmond Community Health, including the conference series Where the Mind Meets the Mouth. Rachel runs the Victorian research study on the Hall Technique and is an inaugural member of the Australian Chapter of the international Alliance for a Cavity Free Future. She also sits on the Dental Board of Australia, Victorian Registration and Notification committee (AHPRA). Ms Alyson Bettega is lead oral health therapist with North Richmond Community Health Oral Health program. She graduated from The University of Queensland in 2007, and completed postgraduate studies in advanced clinical practice at the University of Melbourne. Alyson has experience in remote and rural aboriginal health, and works in clinical teaching and training with North Richmond Community Health and RMIT University. She has a strong interest in MID, and works in the Hall Technique research project.

Professor Christopher Peck, A/Professor Neil Savage & Dr Simon Moore

Professor Christopher Peck Professor Christopher Peck is Dean of the Faculty of Dentistry, the University of Sydney and President of the Australian and New Zealand Academy of Orofacial Pain. Professor Peck manages complex orofacial pain and disorder patients and conducts research trying to understand better the transition from acute to chronic pain, evidence-based diagnosis and management of orofacial pain, and the biomechanics of jaw dysfunction. A strong advocate for multidisciplinary patient management, he is internationally renowned for his leadership and contributions to translational research in orofacial pain and leading the clinical arm of projects involving researchers from biomedicine to mathematics. He is a senior facial pain expert in the NSW public health care system at the Royal North Shore Hospital’s Pain Management Research Institute, involved in patient care, clinical supervision and research. Professor Peck is President of the International Research Diagnostic Criteria for Temporomandibular Disorders Consortium Network, which develops reliable and valid diagnostic criteria for facial pain complaints. Other clinical pain research groups have emulated this consortium model. Additionally, he chaired an expert International Working Group to expand the taxonomy of temporomandibular disorders (Journal of Oral Rehabilitation 2014). A/Professor Neil Savage Dr Neil Savage is registered in the specialties of Oral Medicine and Oral Pathology. His main commitment is in private specialist Oral Medicine practice. He is a Consultant in Oral Medicine and Oral Pathology to the RBWH, conducting a weekly Outpatient’s clinic and participating in registrar training as well as undergraduate Dental and Medical students. He is actively involved in diagnostic histopathology reporting through Queensland Medical Laboratory and consults on difficult cases with a number of interstate pathology groups. He also continues teaching commitments at both post-graduate and under-graduate levels as a Reader in Oral Medicine and Oral Pathology and is an Associate Professor at the University of Queensland, School of Dentistry. He is a member of both the ADA Inc and AHPRA Therapeutics committees and a contributing author to the Therapeutic Guidelines book “Oral and Dental”. He has specific clinical and research interests in the management of soft tissue lesions, both conservative and surgical, the use of steroid medications and oral dysaesthesias. Dr Simon Moore Dr Simon Moore holds dual registration with the Australian Dental Board as a registered specialist within the field of Oral Medicine and Oral Pathology. Following graduation from the University of Adelaide in 1985, Simon served as a Dental Officer in Armed Forces. He then undertook further study in Theology before returning to Adelaide to obtain his MDS in Oral Pathology. Simon has a passion for teaching and was on the staff at the University of Adelaide University until moving to Brisbane in 2004 where he currently works in Specialist Oral Medicine practices in Brisbane and the Sunshine Coast. He also has a heart for those less fortunate and has been involved in dental training programs within East Timor and Cambodia.

Session 1: Getting Started Dates: Friday, 18 May - Sunday 20 May 2018 The course begins with information on basic diagnosis, case selection and treatment planning. Emphasis will be placed on thorough record examination and cephalametric analysis, and their role in making informed treatment decisions. The effects of orthodontic treatment on facial aesthetics and balance will be discussed as well as the basics of craniofacial growth and development. Banding and bonding will be discussed and participants will have the opportunity to place bands and brackets on the orthodontic typodont. Initial wire placement and ligation techniques will be demonstrated. Hands-on exercises include •Cephalometric analysis •Bonding (bracket placement exercise) •Mechanics (wire placement and technique) Session 2: Class I Survey & Biomechanics Dates: Friday, 6 July - Sunday 8 July 2018 This portion of the course will address specific information on treatment timing issues, when to treat and when not to treat. Phase 1 and interceptive treatment modalities and arch development will be discussed. Class I crowded cases with expansion mechanics/appliances and their adjustment will be covered. Participants will then begin a detailed study of basic orthodontic biomechanics. Intrusion and advancing arch wire fabrication and placement with various ligation techniques will be demonstrated. Participants will also begin the discussion of Class II diagnosis and treatment. Hands-on exercises include •Advancing utility arches •Intrusion utility arches (fabrication & adjustments) •Molar uprighting springs (mechanics) Session 3: Class II and Class III Survey Extractions Dates: Friday, 7 September - Sunday 9 September 2018 This session addresses Class II and Class III considerations and treatment modalities, exploring both orthopaedic and orthodontic movements. Topics to be covered include Class II correction in mixed and permanent dentition, (including various molar distalising techniques – fixed and removable), archwire selection, sequencing, elastic wear and set-up, adjunctive intrusion techniques, with particular attention to second and third order bends (tipping and torque). Hands-on exercises include •Torquing arches •Second order bends •Cervical headgear placement and adjustment •Various Class II correctors and their adjustment Session 4: Extraction Mechanics, Finishing, Detailing and Aesthetic Considerations Dates: Friday, 2 November - Sunday 4 November 2018 This session begins with extraction mechanics and their timing as well as focus on the all important topic of finishing and detailing. Included is discussion on the use of elastics and segmented arches for finishing, intraoral adjustments for efficiency, troubleshooting difficult cases and debanding. Aesthetic considerations, ortho-prosthodontic cases, retainer design and adjustments will also be covered including placement of fixed retainers. Dr Eric Lowenhaupt attended Florida State University for undergraduate studies and Emory University in Atlanta for his Dental and Orthodontic training. He completed a general practice residency and practiced general dentistry for six years before returning for his graduate orthodontic training at Emory University. Since 1986, he has been in the private practice of Orthodontics and Dentofacial Orthopedics in Florida. Eric lectures nationally and internationally through his affiliation with (G) nathos (The Ricketts School for Bioprogressive Education, Southeast)and VOCA (Vital Orthodontic Considerations Australasia).
